Thomas L. Zajac, 56, Hoxie, died Tuesday, Aug. 21, 2012, at Via Christi St. Francis Medical Center, Wichita.

He was born May 2, 1956, in Fontana, Calif., to Edward Thomas and Dorothy Louise Zajac.

He married Teresa (Cox) Zajac on March 16, 1979, in California.

Survivors include his wife, of the home; four sons, William Lyons, Hoxie, Daniel Zajac and Stephen Zajac, both of Manhattan, and Jason Zajac, Englewood, Colo.; and a sister, Deb Borger, Quinter.

Services will be at 10 a.m. Saturday at Mickey-Leopold Funeral Home, Hoxie; private family inurnment.

There will be no visitation.
